Epidemiological data indicate that HIV and AIDS are disproportionately affecting American Indians. Specific to American Indian men identifying as gay, bisexual, two-spirit or who have same-sex experiences, this study assessed HIV-risk behaviours and barriers to testing, prevention and treatment efforts. A rapid assessment model was utilised as an indigenous-supporting research design. Rigour and thoroughness were achieved via multiple validation procedures. Central themes surrounding barriers to HIV prevention included social discrimination, low self-esteem and substance use. Findings suggest the underutilisation of condoms due to ineffective placement and limited availability in popular locations among gay, bisexual and two-spirit individuals. Participants indicated that HIV testing is occurring less frequently and that testing was not available after hours or weekends. Barriers to treatment included a mistrust of the current healthcare system, a perceived lack of support from the Indian Health Service for AIDS care and a lack of transportation to healthcare appointments. Lastly, participants discussed and supported culturally-sensitive treatment services. This study calls attention to the value of an American Indian-specific HIV/AIDS service organisation, the presence of indigenous service providers in the community and culturally-sensitive healthcare providers. |

Men who have sex with men (MSM) and transgender women have recorded exponentially higher HIV prevalence rates in nearly every country where reliable data is available. In low- and middle-income countries, substantial resources for the AIDS response come from the largest bilateral, multilateral, and private philanthropic donors and from a country's contribution to addressing its own epidemic. There is no reliable indication of what overall proportion of these resources ultimately reaches MSM and transgender people. In order to begin filling this knowledge gap and to encourage more effective use of donor investments, the Global Forum on MSM & HIV (MSMGF) was commissioned by the Global Fund to Fight AIDS, Tuberculosis and Malaria to undertake a "big picture" mapping of major HIV investments targeted at MSM and transgender people in low- and middle-income countries. Our objective was to examine major donor investments targeted at these two groups in 2009, but we also recorded relevant information from other years (2008, 2010, and 2011) when available. We additionally reviewed national HIV spending data reported by countries in 2010 to better characterize the flow of domestic and international resources that ultimately reach MSM and transgender people in these contexts. The report indicates that most major bilateral, multilateral, private philanthropic and national government funders do not consistently track their investments targeting MSM and transgender people. |

The International AIDS Conference (IAC), organized by the International AIDS Society (IAS), is the world's premier gathering for HIV professionals and people living with HIV. In the past, numerous advocates and networks, including the Global Forum on MSM & HIV (MSMGF), International Network of People Who Use Drugs (INPUD), the Global Network of Sex Work Projects (NSWP) and key transgender activists have shared increasing concern over the poor program coverage of their respective constituencies at each IAC. This long-standing frustration stems from the discrepancy between IAC programming and on-the-ground realities. This disconnect is particularly concerning when available and accurate epidemiological data reveals the disproportionate burden that HIV is having on these vulnerable groups. In an effort to quantify program coverage of men who have sex with men (MSM), people who use drugs, sexworkers and transgender people at the IAC in 2010, the MSMGF undertook an analysis independent from and unsolicited by the IAS. The analysis features a review of every abstract and sessiona programmed, drawing a distinction between non-exclusive and exclusive coverage of the key populations. By non-exclusive we mean an abstract or a session that focused on two or more key populations. By exclusive, we mean an abstract or a session that focused solely on one key population. Non-exclusive coverage therefore also includes exclusive abstracts and sessions. Our analysis revealed a gross underrepresentation of the key populations across the 2010 conference program. A complete analysis of both non-exclusive and exclusive program coverage is contained in this report. A set of key recommendations are provided to underscore the need for robust leadership, transparency and accountability processes at the level of the Conference Coordinating Committee, (CCC) the highest governing body of the IAC. |

Black men who have sex with men (BMSM) are disproportionately affected by the HIV epidemic, yet few prevention interventions have been developed specifically for them. Recent studies suggest that the Internet is a promising intervention delivery avenue. We describe results from our formative work in developing a theory-based online HIV/STI prevention intervention for young BMSM including focus groups, semistructured interviews, and usability testing. The Intervention, HealthMpowerment.org, was created based on the Institute of Medicine's integrated model of behavior change with extensive input from young BMSM. Key interactive Web site features include live chats, quizzes, personalized health and "hook-up/sex" journals, and decision support tools for assessing risk behaviors. Creating an interactive HIV/sexually transmitted infection web site for BMSM was a complex process requiring many adjustments based on iterative feedback throughout all development stages. Preliminary satisfaction, content acceptability, and usability findings support the use of the Internet to deliver risk reduction messages to young BMSM. |

The increasing prevalence of sexually transmitted infection (STI) and delays in access to sexual health care among men who have sex with men are a major public health concern in the UK. This qualitative study, involving semi-structured interviews with 24 men recruited from a genitourinary medicine clinic in Brighton, UK, explored their views towards the introduction of home sampling kits for STI into clinical practice. Participants had previously self-sampled for rectal and oropharyngeal specimens and completed a survey on the acceptability of self-sampling. Interviews were audio-recorded, transcribed and analyzed using a framework approach. Participants preferred to access home sampling kits from medical venues rather than gay social venues due to privacy concerns and fear of being ridiculed by peers. Perceived societal homophobia led to skepticism towards accessing home sampling kits in commercial venues. Assurance about specimen delivery and receiving test results from clinics was important. Views about using home sampling kits for HIV testing were mixed. Home sampling kits were viewed as an adjunct to clinics, but clinic attendance was preferred if symptomatic. Home sampling kits could be a viable alternative to meet the increasing demand for sexual health services, but to improve the home sampling kit uptake the method of service provision must be culturally sensitive and acceptable. |

Success in addressing HIV and AIDS among men who have sex with men, a key population in the global epidemic, is impeded by homophobia. Homophobia as a barrier to HIV prevention and AIDS treatment is a particularly acute problem in the prison setting. In this qualitative study, we explore HIV and AIDS, stigma and homosexuality in the largest all male prison in Jamaica by conducting iterative in-depth interviews with 25 inmates. Participant narratives unveil a purposeful manipulation of beliefs related to homosexuality that impedes an effective response to HIV and AIDS both in prison and wider society. Findings indicate that homophobia is both a social construction and a tangible tool used to leverage power and a sense of solidarity in a larger political and economic landscape. This use of homophobia may not be unique to Jamaica and is an important issue to address in other low- and middle-income post-colonialist societies. |

In September 2008, the World Health Organization (WHO) held a global consultation on "Prevention and treatment of HIV and other sexually transmitted infections (STI) for men who have sex with men and transgender populations", in Geneva, Switzerland. Recommendations from that global consultation, as well as from WHO regional consultations, called for the need to develop guidance for delivering an evidence-based, package of interventions for implementation by the health sector to prevent and treat HIV and other STIs among MSM and transgender people. The guideline is designed for use by national public health officials and managers of HIV/AIDS and STI programmes, nongovernment organizations (NGOs) including community and civil society organizations, and health workers. It may also be of interest to international funding agencies, the scientific media, health policy-makers and advocates. |

To motivate Dutch MSM to take an HIV-test an effective health promotion program was required. The systematic development of such a program is the central focus of the work that is described in this thesis. The aim of the research presented in this thesis was to describe the application of Intervention Mapping (Bartholomew, Parcel, Kok & Gottlieb, 2006) to the systematic development of an online HIV-prevention intervention to promote HIV-testing among MSM in the Netherlands. As such, the work that is described in the different chapters of this thesis provides insight in the social-cognitive and external determinants related to HIV-testing among this target group. Both quantitative and qualitative research methods have been applied to gain insight therein. The results from this research can be of value for future health promotion activities, specifically in the area of HIV]prevention among MSM in the Netherlands. |

Little is known about men's potential motivations and barriers associated with specific sexually transmitted infections (STI) testing methods. In this study, we examined experiences of self-sampling for ano-rectal STI among men who have sex with men (MSM) in a midwestern community in the U.S. A total of 75 MSM were recruited from community venues throughout Indianapolis, Indiana. Participants completed semi-structured interviews, were asked to obtain ano-rectal self-sample in a private restroom, and were asked open-ended questions about their experiences with ano-rectal self-sampling for STI. Participants included 35 White, 27 Black, and 13 Latino MSM who ranged in age from 18 to 57 years. Regardless of sexual practices, most participants who obtained an ano-rectal self-sample (68/75) reported that the sampling procedure was relatively painless and physically easy. However, regardless of previous receptive anal sex, participants also expressed concerns about the nature of the test (i.e., inserting something into their rectum), which required increased levels of privacy and cleanliness compared to collection of urine samples. Self-sampling proved to be a feasible and acceptable method of collecting ano-rectal STI specimens among MSM. Increased testing for ano-rectal STI among MSM may require addressing the location of sampling and testing sites, existing negative perceptions of ano-rectal self-sampling, and the measures in place to promote privacy and cleanliness. |

In Fiji, very little is known about the social and sexual behaviours of men who have sex with men (MSM) and transgender (TS) persons and how they relate to HIV risk and access HIV prevention services. Although global and regional evidence show that these communities are at higher risk of HIV infection, a well informed and appropriate strategy for this population is lacking. To address this situation and contribute to ongoing national and regional efforts to better respond to HIV and STI (sexually transmitted infection), the AIDS Task Force of Fiji (ATFF) -with technical expertise provided by the New South Wales AIDS Council (ACON), Sydney, Australia and support from the UNDP regional programme- has conducted a research-action project exploring male to male sex, gender identity and HIV transmission risk in urban Fiji. The project recently completed is the first research engaging MSM and TS people in Fiji since 1998 that fully involves the community. |

This global edition toolkit is designed for men who have sex with men (MSM) and MSM supporters who would like to incorporate MSM-related advocacy into their current work. Advocacy is defined here as speaking up, drawing a community's attention to an important issue, and directing decision makers toward a solution. The toolkit is like a cookbook, offering a range of recipes for action that orient participants to core concepts with ideas for building advocacy campaigns wherever they are. It is divided into 3 main sections: (A) an overview of advocacy; (B) a group of 5 topic areas that serve as potential advocacy foci, namely investment, services, stigma and discrimination, research, and civil society and networks; and finally (C) 2 chapters with a special focus on working with international human rights mechanisms and working in hostile environments. Each chapter is followed by a list of related resources in the event that groups would like to pursue the topic in more depth. Similar to other parts of Asia, the HIV epidemic in Nepal is concentrated among a small number of groups, including transgender people, or Metis. This study was conducted to explore the social context of stigma among Metis in Nepal to better understand their risk for HIV. Fourteen in-depth interviews were conducted with Metis in Kathmandu, Nepal. We found that stigma from families leading to rural-urban migration exposed Metis to discrimination from law enforcement, employers and sexual partners, which influenced their risk for HIV. Specific HIV-related risks identified were rape by law enforcement officers, inconsistent condom use and high reported numbers of sexual partners. These data point to an immediate need to work with law enforcement to reduce violence targeting Metis. HIV prevention, housing and employment outreach to Metis in rural areas and those who migrate to urban areas is also needed. Finally, there is a need for more research to determine the prevalence of HIV among Metis, to explore risk within sexual networks and to better understand of the relationship between Metis and their families in order to develop future programmes and interventions. |

HIV-prevention efforts with gay men in relationships frequently omit primary partners. When they are considered, examinations of race/ethnicity are often overlooked despite higher infection rates among gay men of colour. Acknowledging both the need to contextualise the behaviours that may affect HIV risk for gay men of colour and the disproportionate impact of HIV on Latino gay men, the present study utilised semi-structured, qualitative interviews to explore relationship dynamics, sexual agreements and behaviours, safer sex choices and HIV risk among nine Latino gay male couples. All participants were HIV-negative and in concordant negative relationships. Additionally, nearly all reported closed agreements. Analysis revealed participants engaging in four behaviours that may actively reduce their HIV risk: approaching sexual agreements from a practical standpoint, maintaining a high literacy around HIV, having exposure to social support groups for Latino gay men and finding support in their relationship with another Latino gay man. Additional issues are raised where long-term HIV prevention is concerned. Intimate relationships are an important context for understanding both HIV risk and prevention among Latino gay men. Likewise, race/ethnicity provides an indispensable perspective on all research with gay couples. Future prevention efforts with gay men must strive to include both. |

This volume presents the first global economic analysis undertaken to explore the emerging epidemics of HIV among men who have sex with men in low- and middle-income countries. Using a scenario-based approach, the book systematically reviews the available data to investigate and characterize these epidemics and propose evidence-based and rightsaffirming responses. These responses, which are demonstrated here to be cost-effective, constitute both a public health priority and a clear human rights imperative. Written to help governments, public health agencies, donors, and communities better understand and respond to the HIV epidemics among these often hidden and stigmatized populations, The Global HIV Epidemics among MenWho Have Sex with Men brings together reviews of epidemiology and the HIV prevention literature; a novel approach to evaluating interventions in prevention, care and treatment; and modeling, costing, and human rights assessments. The book uses a mathematical model to assess the potential country-level impact of interventions, focusing primarily on Kenya, Peru, Thailand, and Ukraine as examples. In all four countries, greater investment in prevention, treatment, and care for men who have sex with men is shown to improve overall HIV epidemic control. |

The Men's INTernet Study II included a randomized controlled trial to develop and test an Internet-based HIV prevention intervention for U.S men who use the Internet to seek sex with men. In 2008, participants (n = 560) were randomized to an online, interactive, sexual risk-reduction intervention or to a wait list null control. After 3 months, participants in both conditions reported varying degrees of change in sexual beliefs or behaviors. Using content analysis and logistic regression, this mixed-methods study sought to understand why these changes occurred. Level of critical self-reflection of assumptions appeared to facilitate the labeling of sexual beliefs and behaviors as risky, which in turn encouraged men to commit to and enact change. New HIV prevention interventions should include activities in their curriculum that foster critical self-reflection on assumptions. |

Focusing on issues concerning MSM, HIV and human rights at the global level, the MSMGF has developed a list of the 10 most significant policy issues and events of the past year. Although we have chosen to include developments that are not policies in and of themselves, all items have significant HIV policy implications at the global level for MSM. The list is based on the MSMGF's five core operating goals, developed by MSM advocates from around the world serving on the organization's Steering Committee. These goals are: 1) Increased investment (funding) in effective HIV prevention, care, treatment and support programmes for MSM 2) Expanded coverage of (roll out of and access to) quality HIV-related services for MSM 3) Increased knowledge on MSM and HIV through the promotion of research and its broad-based dissemination 4) Decreased stigma, discrimination, and violence against MSM 5) Strengthened and linked regional, sub-regional and nation networks of MSM around the world, including networks of MSM living with HIV |
